Hilltown Community Health Center

Eliza Lake is the CEO of Hilltown Community Health Center (HCHC).  She has been involved with the health center since 2010, first as a board member and then as the Director of Community Services and Grant Writer. She became CEO in July 2014. Previously, she worked as a consultant for ten years to state governments and New England non-profits in the area of long-term services and supports policy, was the Director of Community Supports and Services at the Massachusetts Executive Office of Elder Affairs, and a District Aide for State Representative Stephen Kulik. Eliza has an MSW from Boston University, and an AB from Harvard University.  A native of Worthington, Eliza has been a patient of HCHC since 1981, and moved back to town with her husband and two small children in 2009 to take over her family’s farm, on which they raise beef cattle, hay, maple sap, chickens, and, occasionally, pigs.
