Title: Care Manager, Long Term Support Services Location: Boston

Summary:

 Community Care Cooperative (C3) is a new, 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by FQHCs. Our mission is to leverage the collective strengths of Federally Qualified Health Centers (FQHC) to improve the health and wellness of the people we serve.

 The Care Manager will be responsible for performing comprehensive assessments and care planning for health center patients who have been identified for the MassHealth Long Term Services and Supports Community Partner (LTSS CP) Program.  The individual in this role will work directly with health center patients and ensure that the patient’s medical, behavioral, and social service needs are assessed, and a plan of care is in place, coordinating with a nurse or other medical professional as needed. The Care Manager will be responsible for collaborating closely with the Patient’s primary care team and the assigned LTSS CP provider.  This position is a centralized resource who will support multiple health centers teams.

The Technical Business Analyst will be called upon to understand the business, clinical and operational sides of health care organizations, and use the underlying data to discern patterns, identify opportunities, and support improvements

Title: Community Health Worker,Healthy Connections Program Location: Remote

Summary:

 Community Care Cooperative (C3) is a new, 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by FQHCs. Our mission is to leverage the collective strengths of Federally Qualified Health Centers (FQHC) to improve the health and wellness of the people we serve.

The Community Health Worker will serve a vital role within the newly created Healthy Connections Program.  Healthy Connections is a community-based program serving health center patients with complex medical, behavioral, and social service needs. The Community Health Worker will provide direct patient support through home visiting, attending medical appointments, and assisting with connecting patients to social and support services. This position presents an excellent opportunity for a patient-focused individual to care for complex patients in a community setting.

Title: Nurse Clinical Manager Location: Remote

Summary:

Community Care Cooperative (C3) is a new, 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by FQHCs. Our mission is to leverage the collective strengths of Federally Qualified Health Centers (FQHC) to improve the health and wellness of the people we serve. 

 Community Care Cooperative (C3) is seeking an experienced nurse manager who has case management, utilization management, and staff supervision experience in an integrated behavioral health and medical environment to join the clinical operations team.   This role will report to the Senior Manager of Transitions of Care an would be part of an interdisciplinary team comprised of nurses, social workers and community health workers working with at risk Medicaid members of C3. The person in this role would provide oversight and leadership to the nursing staff both in our transitions of care team and our new home health program, to ensure our patients return safely home after hospitalizations, and that those receiving home health services, are getting the service that meets their needs.
