Community Care Cooperative (C3) is a rapidly growing non-profit created by health centers, for health centers.
Our mission is to leverage the collective strengths of Federally Qualified Health Centers (FQHCs) to improve the health and wellness of the people we serve. Our strategy is to unite FQHCs at scale to strengthen primary care, improve financial performance, and advance racial justice.
It’s a big goal, but an important one.
Driven by a strong sense of community, health centers are dedicated to remaining independent so they can provide unmatched, individualized patient experiences to the people they serve. With a sharp focus on further enhancing our mission and expanding the services we can provide to patients, C3 formed two business affiliates. Community Technology Cooperative (CTC) implements and operates a best-in-class Electronic Health Records system (Epic) and other technologies for healthcare centers. Community Pharmacy Cooperative (CPC) operates on-site pharmacies, improves 340B revenue and compliance, and provides clinical services for FQHC’s and their patients.

CPC delivers clinical and economical value to FQHCs by strengthening pharmacy operations, 340B programs, and clinical services. CPC is a non-profit governed by its health center customers.
