Welcome to Community Care Cooperative (C3)

C3 is an Accountable Care Organization (ACO) serving MassHealth Members across Massachusetts. We also serve Medicare Members in certain areas of both Massachusetts and Louisiana. An ACO is a group of doctors and other health care providers who come together to help care for you. At C3, we work to help your primary care provider stay better informed and involved in your care, both inside and outside the health center or practice. We are creating new and improved programs and systems to get you the right care at the right time. We are focused on providing reliable, convenient, quality care in the community where you live and work.

Your Benefits Stay the Same

With C3, your benefits remain the same. If you are a MassHealth member enrolled in our ACO, or a Medicare member enrolled in our ACO REACH program, you will have the same benefits and receive more. You may see your MassHealth or Medicare providers without any new paperwork or rules.

You Keep Your Health Center Primary Care Provider (PCP)

If you receive care at one of our participating health centers or practices, everything stays the same. You will keep your health center or affiliated practice. Find a PCP who is part of our network.

Access to a Broad Network of Hospitals and Specialists

As a member of C3, you have access to a broad network of hospitals and specialists available to MassHealth and Medicare members. If you are a MassHealth member, you may see any specialist or hospital that accepts MassHealth without any new paperwork or rules. Ready to Join C3

Get the best of community-based care while accessing a broad network of hospitals and specialists. If you are a MassHealth (Medicaid) or Medicare member, you may choose from one of our programs. You may benefit from care management and other services that we offer.

MASSHEALTH MEMBER ELIGIBILITY

You are eligible for the C3 MassHealth ACO enrollment if you must:

Have MassHealth Standard, CarePlus, CommonHealth, or Family Assistance plan types

Be younger than
65 years of age

Have no other health insurance

Reside in the community (not in a nursing facility or chronic inpatient hospital)

MEDICARE ACO REACH MEMBER ELIGIBILITY

To be eligible, patients must…

Beneficiary must be enrolled in Medicare
Parts A and B

Medicare is the
primary payer

Beneficiary must be resident of the U.S.

Beneficiary is not covered under Medicare Advantage or other Medicare managed care plan

Beneficiary must reside in a county that is included in the ACO’s service area (counties in which your centers are located and adjacent counties are included in the service area)

Selecting your provider and ACO

You may choose to align our ACO voluntarily by designating a Participant Provider affiliated with C3 as their primary clinician or main source of care. It is your choice to choose the health care providers with whom you want to have a care relationship.

When you choose a primary clinician that participates in our ACO REACH, they get access to tools and services to better coordinate your care and improve your health. Your Medicare benefits stay the same.

To Indicate Your Primary Clinician, you may visit the Medicare Find and Compare Health Care Providers and follow the directions under “Add your favorite providers.” Or you may call 1-800-MEDICARE (TTY users should call )1-877-486-2048

Print the PCP Selection Form and mail or fax it to C3 or your provider.

MEDICARE MSSP MEMBER ELIGIBILITY

To be eligible, patients must…

Beneficiary must be enrolled in Medicare
Parts A and B

Medicare is the
primary payer

Beneficiary must be resident of the U.S.

Beneficiary is not covered under Medicare Advantage or other Medicare managed care plan

Beneficiary must reside in a county that is included in the ACO’s service area (counties in which your centers are located and adjacent counties are included in the service area)

Beneficiary must have had at least one primary care service with a Primary Care Physician (PCP) who is a part of our ACO and received the plurality of primary care services from our ACO.
