Jonathan Liebman, ANP
Medical Director, Hilltown Community Health Center
Jon Liebman, Medical Director of Hilltown Community Health Center, is a board certified Adult Nurse Practitioner and has over thirty years of experience in public health, primary care and health care administration. Mr. Liebman received his undergraduate degree in Urban Studies from Columbia University in New York City, and began his career as an urban planner involved in housing and community development. He subsequently received an M.S. in Health Policy and Management from the Harvard School of Public Health.
As a member of the research staff of Philadelphia’s Public Health Management Corporation, he directed HIV prevention and research programs targeting sex workers and injection drug users, as well as other health policy studies. As a National Health Service Corps scholar, he graduated from Yale University with an M.S. in Nursing in 1998 and began a new career as a primary care provider in Massachusetts. He spent over 15 years as a clinician at Holyoke Health Center, where he directed that center’s diabetes care and HIV programs, including serving as the program director for the regional Ryan White HIV Care Program in Holyoke and Springfield. He also worked as a primary care clinician at the Baystate Brightwood Health Center in Springfield.
In 2014 he joined the staff of Hilltown Community Health Centers as a clinician and manager of the quality improvement program, and subsequently became Medical Director. In that role he oversees the clinical operations of two rural primary care medical clinics and a school-based health center.
