Your Medicare Benefits Will Remain the Same

ACO REACH stands for a Accountable Care Organization (ACO) Realizing Equity, Access, and Community Health. Medicare has started an initiative that enables health care providers who share the common goal of improving patient care to work together more effectively. Health care professionals in our ACO are now able to work together with Medicare to give you more coordinated care and services.

Benefits

If you were aligned with C3 by Medicare or you have chosen to be aligned with our ACO REACH, your Medicare benefits will stay the same. You can continue seeing any Medicare health care provider you choose.

Member ID Cards

You will not receive an additional ID card and all services will be paid for by Medicare. You should continue to use your Medicare beneficiary ID card.

Member Alignment

ACO REACH stands for an Accountable Care Organization (ACO) Realizing Equity, Access, and Community Health. This is a Medicare ACO model that aims to improve patient care.

To be eligible for our ACO REACH program, you must:

Be enrolled in Medicare Parts A and B
Have Medicare as the primary payer
Not be covered under Medicare Advantage or other Medicare managed care plan
Be resident of the United States
Reside in a county that is included in our ACO’s service area

Selecting Your Health Care Provider and ACO

You may choose to join our ACO by designating a Participant Provider aﬃliated with C3 as your primary clinician or main source of care. It is your choice to choose the health care providers with whom you want to have a care relationship.

When you choose a primary clinician that participates in our ACO REACH, they get access to tools and services to better coordinate your care and improve your health. Your Medicare beneﬁts stay the same.
