About Us
Community Care Cooperative (C3) exists to deliver great health care to all by offering a series of health plans uniquely tailored to meet each individual’s medical needs, no matter what road they are on in their healthcare journey. We work hand-in-hand with our provider partners, so they stay better informed and involved in our members’ care, both inside and outside the health center or practice. We also regularly collaborate with our members’ physicians and care teams, so we can build innovative programs and enhance existing systems that get our members the care they need, when they need it most.
C3 is the only national non-profit Accountable Care Organization founded and governed by its Federally Qualified Health Center partners. With a network of 1,500 primary care physicians, and 3,200 clinicians working in Massachusetts, D.C., California, Connecticut, Louisiana, North Carolina, Oregon, and Washington state, we are intensely focused on providing dependable, convenient, and quality care in the communities where our members have chosen to live, work, and raise their families.

Within our vision of transforming the health of underserved communities, we aim to improve the health and wellness of the people we serve and understand all factors that contribute to their health.

Our Work
The core of our work is the design and implementation of strategies that advance integrated and coordinated community-based care. We support FQHCs in this goal through our wide range of services:
- Accountable Care Organization participating in MassHealth Medicaid and Medicare ACO REACH
- Pharmacy services including 340b pharmacy conversion support and establishing/reestablishing on-site health center pharmacies
- Technology services with speciality focus on implementing the best-in-class Epic Electronic Health Record
- Telehealth integration into primary care with a comprehensive playbook
- Consulting services
- Other shared services including risk coding, project management, and more
Our proven approach works as a model of best-in-class patient care and a savvy business strategy. We achieved this efficiency through supporting advanced and holistic primary care at their health centers, as well as providing training, IT, quality, and practice improvement programs and strengthening relationships with patients and their communities.
We Are C3
Our leadership team brings vast experience in the areas of managed care, delivery systems, health insurance, Accountable Care Organizations, quality, public policy, and public health. Our innovative, multi-disciplinary team is uniquely positioned to advance comprehensive primary care strategies and integrated delivery systems to better serve our complex populations and FQHCs.
FQHCs exist to serve under-resourced and low income populations, which is why health equity and racial justice are at the heart of everything we do. We understand the distinct and diverse needs of our communities and are invested in their wellbeing and growth. Within our Board of Directors’ resolution outlining our commitments, we hold ourselves accountable to serving ethnically diverse patient populations in both urban and rural regions. To do so, we focus on addressing physical, behavioral, and social health and pursue health equity in care through our variety of programs and initiatives.

Community Technology Cooperative (CTC) implements and operates a best-in-class Electronic Health Records system (Epic) and other technologies for FQHCs.

Community Pharmacy Cooperative (CPC) operates on-site pharmacies, improves 340B revenue and compliance, and provides clinical services for FQHCs and their patients.

Founded in March 2020 by Community Care Cooperative and the Massachusetts League of Community Health Centers, the FQHC Telehealth Consortium helps health centers across Massachusetts pivot to sustainable models of telehealth, integrate them into their primary and behavioral care practices, and ensure financial viability.
Community Care Cooperative (C3) is a 501(c)(3) non-profit Accountable Care Organization (ACO) governed by FQHCs. Our mission is to leverage the collective strengths of Federally Qualified Health Centers (FQHC) to improve the health and wellness of the people we serve.
Shared savings or losses information and quality measures for Performance Year 2023
will be posted once it becomes available.
There are no joint ventures between or among our ACO and our Participant Providers.
