Title: Community Health Worker,Healthy Connections Program Location: Remote

Summary:

 Community Care Cooperative (C3) is a new, 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by FQHCs. Our mission is to leverage the collective strengths of Federally Qualified Health Centers (FQHC) to improve the health and wellness of the people we serve.

The Community Health Worker will serve a vital role within the newly created Healthy Connections Program.  Healthy Connections is a community-based program serving health center patients with complex medical, behavioral, and social service needs. The Community Health Worker will provide direct patient support through home visiting, attending medical appointments, and assisting with connecting patients to social and support services. This position presents an excellent opportunity for a patient-focused individual to care for complex patients in a community setting.

Title: Population Health Manager Location : North Shore of Boston

Summary:

Under the direction of the Chief Operations Office or her designee, and in close collaboration with the C3 Health Center Performance Management Team and the C3 Office of Practice Transformation, you as the Population Health Manager (PHM) will be responsible for driving value-based care initiatives at North Shore Community Health. Partnering closely with North Shore Community Health leadership, providers, and staff, you will integrate C3’s clinical programs and resources into the practice workflows with the intent to optimize the enrollee experience, and positively impact provider engagement and quality of care provided. 

In addition, approximately 20-25% of your time will be devoted to central C3 projects, including work supporting the Health Center Performance Team as well as engagement and onboarding of new program initiatives.

Title: Health Center Performance Manager Location: Western Region

Summary:

Community Care Cooperative (C3) is a new, 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by FQHCs. Our mission is to leverage the collective strengths of Federally Qualified Health Centers (FQHC) to improve the health and wellness of the people we serve.

The Health Center Performance Manager will serve as the primary relationship manager to the health centers providing oversight and assistance to support health centers in achieving their annual cost and quality goals.  The position will also support other required functions, such as regulatory requirements of ACO contracts.     

The job is a true generalist; the position requires enough subject matter expertise in the full scope of our organization’s business operations to be able to trouble shoot, diagnose and fix a broad array of issues.  When not able to directly resolve issues, the Performance Manager must be skilled enough to trouble-shoot who to go to for additional support and keep accountability for ensuring that all open issues get resolved.  The scope of these generalist functions includes, but is not limited to, Complex Care Management, Transitions of Care, Coding, Quality, Practice Transformation, use of DSRIP, and EHR optimization. This person must exhibit strong leadership skills, organization and communication skills, and a strong attention to detail.

While this role will have a defined set of Health Center locations, the job will require visiting health center locations throughout the state of Massachusetts, and required time at the organizations office.

Title: Nurse Clinical Manager Location: Remote

Summary:

Community Care Cooperative (C3) is a new, 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by FQHCs. Our mission is to leverage the collective strengths of Federally Qualified Health Centers (FQHC) to improve the health and wellness of the people we serve. 

 Community Care Cooperative (C3) is seeking an experienced nurse manager who has case management, utilization management, and staff supervision experience in an integrated behavioral health and medical environment to join the clinical operations team.   This role will report to the Senior Manager of Transitions of Care an would be part of an interdisciplinary team comprised of nurses, social workers and community health workers working with at risk Medicaid members of C3. The person in this role would provide oversight and leadership to the nursing staff both in our transitions of care team and our new home health program, to ensure our patients return safely home after hospitalizations, and that those receiving home health services, are getting the service that meets their needs.
