Title:  Health Center Performance Manager Location: Western Mass

Summary:

Community Care Cooperative (C3) is a new, 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by FQHCs. Our mission is to leverage the collective strengths of Federally Qualified Health Centers (FQHC) to improve the health and wellness of the people we serve.

The Health Center Performance Manager will serve as the primary relationship manager to the health centers providing oversight and assistance to support health centers in achieving their annual cost and quality goals.  The position will also support other required functions, such as regulatory requirements of ACO contracts.     

The job is a true generalist; the position requires enough subject matter expertise in the full scope of our organization’s business operations to be able to trouble shoot, diagnose and fix a broad array of issues.When not able to directly resolve issues, the Performance Manager must be skilled enough to trouble-shoot who to go to for additional support and keep accountability for ensuring that all open issues get resolved.The scope of these generalist functions includes, but is not limited to, Complex Care Management, Transitions of Care, Coding, Quality, Practice Transformation, use of DSRIP, and EHR optimization. This person must exhibit strong leadership skills, organization and communication skills, and a strong attention to detail.

While this role will have a defined set of Health Center locations, the job will require visiting health center locations throughout the state of Massachusetts, and required time at the organizations office.

Title: Community Health Worker, Healthy Connections Program Location: Boston

Summary:

 Community Care Cooperative (C3) is a new, 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by FQHCs. Our mission is to leverage the collective strengths of Federally Qualified Health Centers (FQHC) to improve the health and wellness of the people we serve.

The Community Health Worker will serve a vital role within the newly created Healthy Connections Program.  Healthy Connections is a community-based program serving health center patients with complex medical, behavioral, and social service needs. The Community Health Worker will provide direct patient support through home visiting, attending medical appointments, and assisting with connecting patients to social and support services. This position presents an excellent opportunity for a patient-focused individual to care for complex patients in a community setting.

Title: Director, Social Determinants of Health Location: Boston

Summary:

 Community Care Cooperative (C3) is a 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by Federally Quality Health Centers (FQHC). Our mission is to leverage the collective strengths of FQHCs to improve the health and wellness of the people we serve.

 It is core to our mission and our goals in our contract with MassHealth to address the health related social needs of our members and have an impact on the social determinants of health of the population we serve.  The Director for Social Determinants of Health (SDOH) will provide leadership for this important work.  S/he is responsible for developing, implementing and managing strategies for SDOH through all aspects of the model of care and the delivery system.  The individual will work throughout the Company, with participating health centers, and with the MassHealth network of providers to facilitate efforts to achieve goals for quality care and total cost of care for our members with SDOH needs. In addition, this role will provide leadership in the development and operation of the Flexible Services Program. The Director of SDOH reports to the Chief of Practice Transformation and Quality.

 The Director of SDOH is part of the Office of Practice Transformation and Quality (PTQ). The goal of the Office of PTQ is to enable and support participating FQHCs to advance and sustain the Integrated Patient-Centered Medical Home (PCMH) model. We aim to achieve excellence in a value-based ACO infrastructure, enhance quality of care, and to bend the cost curve and support the long-term sustainability of the MassHealth program. We strive to exceed our cost and quality goals in all ventures.
